 ONE APPLICATION FOR ABOUT 30 SCHOLARSHIPS! All you have to do is find the scholarship you would like to apply to. Fill out all required pieces to the application and print the same form – replacing the name and number of the scholarship. Make sure to ask Ms Phillips for copies of your transcript.

 ·      FILL OUT ALL REQUIRED PIECES TO THE APPLICATION

 ·      PRINT THE SAME FORM – REPLACING SIMPLY THE NAME AND NUMBER OF THE SCHOLARSHIP!

 Complete application includes:

• Release of Information – Must be included for each scholarship (Must be signed by parent regardless of your age).
• PHS Application – required for all scholarships
• Financial Application – only if required for specific scholarship (noted by *)
• Recommendation 1– required for all scholarships
• Recommendation 2 - required for all scholarships
• Resume Forms – required for all scholarships
• Essay (noted by **)
• An unofficial transcripts must be included for each scholarship (pick it up from MS. Phillips)
